When you walk through our doors you will be welcomed as part of the family. On your fist visit we will ask you to fill out some paperwork so we may gather important history about your hearing development.
It is very important that you bring all of your health insurance cards to your first appointment. Examples are Medicaid Card, Medicare Card, Medicare Supplement, or Private Insurance Card. If you would like to have your testing submitted to insurance, we must have a referral from your primary doctor in our office on or before the day of testing.
